I am often in meetings with clients so in order to expedite the intake process, please leave the information noted above by secure voicemail or e-mail and I will follow up by sending you a link to a secure client portal where you can review and complete all of the necessary intake paperwork.

I accept many insurances listed below.  However, because there are various plans within each insurance carrier, it is always a good idea to call the number on the back of your card to verify that I am an in network provider for your specific plan and what your responsibility is for deductibles and copays.
